Payment Contract
Every tick confirms that you have accepted our T&C's moving forward and that these will be adhered to.
This section must be filled in by the person responsible for the school fees.
Payment of Fees
I hereby assume absolute responsibility for the payment of any fees and charges that may fall due as a result of the admittance to Kiddies Christian Academy (Pty) Ltd,
of the child whose details appear on this enrolment form.
I acknowledge that school fees are payable in advance by the 1st day of each month, from 1st January to 1st December,
and that there are various payment options offered by Kiddies Christian Academy (Pty) Ltd.
I acknowledge that a months’ notice is required in writing or a month’s school fees are required in lieu of notice,
if a child is withdrawn from Kiddies Christian Academy (Pty) Ltd. If a child leaves the school during the month for any reason,
whatsoever either at the insistence of Kiddies Christian Academy (Pty) Ltd or Parents, the balance of the school fees for that month,
will be forfeited or a month’s school fees in lieu of notice will be payable.
I acknowledge that should any one installment payable in the month hereof not be paid on due-date,
then the balance outstanding at the date of the next statement, shall immediately become due and payable by me and that no indulgence or grant of time by
Kiddies Christian Academy (Pty) Ltd shall be deemed a waiver of its rights hereunder.
In addition, interest will be charged on the balance outstanding at a rate of 10%.
I choose the residential address set out above as my domicillium citandi et executandi for the service on me by
Kiddies Christian Academy (Pty) Ltd of all notices, processes and other communications.
I agree and understand that:
School fee refunds will only be given if parents/guardians are withdrawing the child from Kiddies Christian Academy (Pty) Ltd
after the appropriate notice has been given.
That should I not meet the terms of payment required,
I might be asked to keep the child at home until the arrears and any interest charged is paid.
Accounts that are continually ignored will be handed over to our Debt Collectors for the recovery thereof.
All costs incurred thereafter, irrespective of whether or not Kiddies Christian Academy (Pty) Ltd’s claim is settled before proceedings are instituted,
will be for my own account.
Section 6
Indemnity Form
Even though the nursery school will take every possible precaution to safeguard your child,
we do not hold ourselves responsible for any accidents which may occur while the child is in our care at the nursery school.
I, the undersigned, herewith apply for the enrollment of my child,
to Kiddies Christian Academy (PTY) LTD and I undertake the following:
I, as the natural guardian of the above named child on behalf of myself, my assigns, heirs and executors,
hereby indemnify the owners and employees of KIDDIES CHRISTIAN ACADEMY (PTY) LTD from any liability or damage whatsoever and any legal expenses
or costs which may arise from any claim as a result of the death of the above child arising from sickness or of injury which the said child might have contracted
or sustained during their sojourn in the centre, except where such injury,
illness or damage is as a result of the unlawful and intentional negligence of the school or an employee of the said school.
I undertake to have the above child immunised against all infectious diseases and children’s diseases at admission.
I, as the natural guardian of the above named child do hereby state that I will not hold the owners or employees of
KIDDIES CHRISTIAN ACADEMY (PTY) LTD responsible for any accident that may occur while my child is in transit, except where such injury,
illness or damage is a result of the unlawful and intentional negligence of the school or an employee of the said school.
I, the undersigned, further undertake to abide by the rules laid down by KIDDIES CHRISTIAN ACADEMY (PTY) LTD
or which may be issued from time to time.
In addition to the above understanding,
I grant the principal and/or staff my consent to obtain whatever medical treatment might be necessary during an emergency where I shall not be immediately
or timeously able to grant consent in person.
I understand that my child may be photographed at Kiddies Christian Academy (Pty) Ltd during normal school hours, field trips,
or activities. I understand that these photographs may be used in promoting childcare services, either in print or on the Internet.
I grant permission for my child to be photographed, or their images recorded for print or electronic use in promoting
Kiddies Christian Academy’s services. I understand that there will be no payment for me or my child’s participation in this release.
